· Licensed Registered Nurse in good standing.

· Geriatric nursing background preferred.

· Working knowledge of nursing home regulations.

· Ability to work independently as well as a meaningful member of the team.

· Excellent organizational skills in order to coordinate many projects simultaneously.

· Ability and willingness to set goals, monitor progress and obtain established goals within the established time frames.

· Genuine caring for and interest in elderly and handicapped people.

· Schedule residents for appropriate admission, annual, quarterly, Medicare assessment or change-of-condition assessments, and oversee communication to nursing and other departments.

· Assure compliance with Federal and State regulations regarding completion / coordination of the comprehensive assessment (MDS).

· Monitor MDS and care plan documentation on all residents, assuring that complete, orderly chronological and current documentation is in file.

· Maintain proficiency in software. (We use PCC)

· Coordinate and attend regular Medicaid meetings to discuss current residents, pending residents and recipients.

· Coordinate and attend all meetings in order to gather information, communicate changes, maintain and update the clinical records including the care plan.

· Coordinate Level of Care Meetings for clinical and participate in financial LOC meetings.

· Perform care plan updates as needed.

· Participate in Triple Check Meetings.

· Provide DON, Administrator and pertinent staff with the QI/QM report and coordinates access to Software for team members to access own reports.

· Perform transmissions of the MDS on a timely basis.

· Remain current with changes in the industry through attendance at outside workshops and conferences.

· Manage the Therapy Caseload and address residents’ therapy needs daily.

· Assist with ABN’s; Physician Certification and Recertification’s.

· Oversee Medicare Part A Consolidated Billing Letters for Hospital, Physicians, and Ambulance.

· Manage Electronic Kiosk POC systems including training new and existing staff.

· Coordinate Managed Care authorization process.

· Coordinate the HHSC-OIG RUG Audits.

· Monitor coding on QI/QM.

· Review of ADR documentation.

· Perform Other duties as required.

Job Type: Full-time

Pay: $68,000.00 - $80,000.00 per year
